Communify Queensland Ltd is committed to protecting your privacy. We comply with the Privacy Act 1988 (Cth), including the Australian Privacy Principles, and all relevant legislation.
This Privacy Collection Notice outlines how and why we collect, use, store, and share your personal and sensitive information, and your rights in relation to that information. Collecting Personal Information
Communify will only collect personal information about you with your permission except where the law requires or allows. We will only collect necessary information about you that will enable us to provide you with requested supports and services. Information will be collected from you, and with your consent, from other stakeholders such as family members, carers, other support services and government agencies. We will keep you informed about the types of information we collect about you and how it will be used.
You are not required to give consent or provide the personal information that Communify requests. However, if this choice is made, Communify may not be able to provide you with some or all of our services.

Why we collect personal information
We may collect information about you that will enable us to:
- Determine your eligibility for services and assess your support needs
- Provide tailored care, referrals, and community support
- Coordinate with other key people, professionals or agencies supporting you
- Contact you or your authorised representative in urgent situations
- Meet our legal obligations and reporting requirements (e.g. to government funders)
- Improve service delivery through quality improvement and internal reporting
- Conduct research and evaluation (with your information de-identified where possible).
Who has access to your information?
Your information is only accessible to authorised Communify Qld staff. We will not use or disclose any information about you for other purposes without consent, unless:
- Required or authorised by law.
- There is a serious threat to your or another person’s health or safety
Protecting your information
At Communify, we are committed to safeguarding your personal information from misuse, interference, loss, unauthorised access, modification, or disclosure. Your privacy is protected by law, including the Privacy Act 1988 (Cth) and relevant state legislation. We store hard copy records in locked, secure filing systems. Electronic information is stored in access-controlled databases and document management systems, protected through restricted permissions and cyber security safeguards. When your information is no longer required, we dispose of or archive it securely in
accordance with the Queensland State Archives guidelines and applicable legal and regulatory requirements.
Your rights
At any time you have the right to contact Communify in person, by email, phone or letter to:
- Request access to the personal information we hold about you.
- Request correction of any inaccurate or incomplete personal information we hold about you.
- Withdraw or limit your consent (please note this may impact the services we can provide).
- Ask questions or make a complaint about how your privacy has been managed.
We will review your request and notify you of the outcome within a reasonable timeframe.

You may also contact the Office of the Australian Information Commissioner (OAIC) on 1300 363 922 if you are not satisfied with our response to your complaint, or you consider that Communify may have breached the Australian Privacy Principles or the Privacy Act.
